PARTICIPANT WAIVER AND RELEASE

I agree to pay the full subscription of {membership_fees}. In joining, members subscribe to Haverhill Inner City Boxing 's policies and are expected to abide by these rules. We have the right to remove your privileges at Haverhill Inner City Boxing if our policies are compromised

EFT MEMBERS are authorizing Haverhill Inner City Boxing to electronically deduct your dues and other miscellaneous charges made at Haverhill Inner City Boxing from your bank account or credit card. Dues are deducted at the end of each month and are applicable for the following month. EFT MEMBERSHIPS are a month-to-month membership and will continue unless Haverhill Inner City Boxing receives notice of cancellation. If members want to pay at our facility or cancel their membership instead of being drafted, they must do so at least 5 business days prior to their payment due date.

Decline/Insufficient Funds: If Haverhill Inner City Boxing cannot deduct your dues by the first of the month due to your credit card decline or insufficient funds, there will be a fee of $15.00 applied to your account. Haverhill Inner City Boxing will then try to bill you 10 days later and if payment is rejected we will try again later in the month. If a member is delinquent for more than two months membership will be cancelled and if a member chooses to renew membership another enrollment fee will be applied.

WAIVER AND RELEASE OF LIABILITY

It is understood and agreed that THE MEMBER has his/her physician's permission to engage in physical exercise and/or tanning and that Haverhill Inner City Boxing does not make any warranties or representations with respect ot the advisability/non-advisability of the THE MEMBER to engage in such exercise and/or tanning. THE MEMBER does hereby remise, release, and forever discharge Seller of and from all actions and causes of actions, suits, claims, and demands arising out of injuries sustained on Seller's promises.

Express assumption of risk: I, the undersigned, am aware that there are significant risks involved in all aspects of physical training. These risks include, but are not limited to: falls which can result in serious injury or death; injury or death due to negligence on the part of myself, my training partner, or other people around me; injury or death due to improper use or failure of equipment; strains and sprains. I am aware that any of these above mentioned risks may result in serious injury or death to myself and or my partners). I willingly assume full responsibility for the risks that I am exposing myself to and accept full responsibility for any injury or death that may result from participation in any activity or class while at, or under direction of Haverhill Inner City Boxing .

I acknowledge that I have no physical impairments, injuries, or illnesses that will endanger me or others.

Release: In consideration of the above mentioned risks and hazards and in consideration of the fact that I am willingly and voluntarily participating in the activities offered by Haverhill Inner City Boxing . I, the undersigned hereby release Haverhill Inner City Boxing their principals, agents, employees, and volunteers from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with my participation in this activity, including those allegedly attributed the negligent acts or omissions of the above mentioned parties. This agreement shall be binding upon me, my successors, representatives, heirs, executors, assigns, or transferees. If any portion of this agreement is held invalid, I agree that the remainder of the agreement shall remain in full legal force and effect.

If I am signing on behalf of a minor child, I also give full permission for any person connected with Haverhill Inner City Boxing to administer first aid deemed necessary, and in case of serious illness or injury, I give permission to call for medical and or surgical care for the child and to transport the child to a medical facility deemed necessary for the wellbeing of the child.

Indemnification: The participant recognizes that there is risk involved in the types of activities offered by Haverhill Inner City Boxing . Therefore the participant accepts financial responsibility for any injury that the participant may cause either to him/herself or

In consideration of the opportunity to participate in the Activities, and in recognition of the need for Haverhill Inner City Boxing Club, as a nonprofit corporation, to solicit donations, raise funds, and market its services, I further authorize and grant ot Haverhill Inner City Boxing Club, any right and permission to use, copyright, publish and re-publish my image, likeness, appearance and voice in the forms of photograph, video, sound-clip, interview or any other form, ni the same, in whole or ni part, in conjunction to any matter related to Haverhill Inner City Boxing Club and for any purpose whatsoever. Iunderstand that my image, likeness, appearance and voice may be used for illustration, promotion and advertising or for any other purpose. I further grant to Haverhill Inner City Boxing Club the right to produce or reproduce my image, likeness, appearance and voice on television, radio, satellite, internet, print or any other form of media. I hereby also grant to Haverhill Inner City Boxing Club the right to transfer, use or cause to be used, my image, likeness, appearance and voice. Neither have I otherwise been compensated nor will I seek at any time compensation for any use of my image, likeness, performance, appearance and/or voice. I understand that all photographs, videos, sound-clips, interviews or any other form of media (including electronic media whether now known or hereafter created) taken of me is and will be the sole property of Haverhill Inner City Boxing Club. I irrevocably release and discharge Haverhill Inner City Boxing Club from any claims of invasion of privacy and/or right to publicity and/or copyright infringement and/or any intellectual property rights claim of any kind whatsoever.
